Chronic Illness Management Subscription
As we continue to grow and improve the delivery of essential healthcare to our patients, new systems are gradually being put in place guarantee high-level success. As of August 25th, we will have our Monthly chronic illness subscription plans available for those who wish to subscribe for such services.
Emphasis will be placed on Hypertension and Diabetes Mellitus. For Hypertension: - On demand Blood pressure monitoring with one (1) graphical BP report at the end of the month. - Four (4) in-clinic Blood Pressure checks if needed. - One (1) refill of current Hypertensive medications. - One (1) House call. - Two (2) scheduled virtual consults. All at an 18,000 package fee.
For Diabetes -On demand Blood sugar monitoring with one (1) graphical RBS report at the end of the month. - Four (4) in - clinic RBS tests if needed. - One (1) refill of our prescription Diabetic medications. -One (1) house call. - Two (2) scheduled virtual consults. All at an 18,000 package fee.
Scheduling appointments and payments can both be done online and via MMG respectively so we encourage our clients to indulge in the convenience we offer.
